The 5-digit extension code refers to the 5-digit barcode appended to the end of the ordinary barcode. The picture below shows the barcode with the 5-digit extension code. The blue line on the left is the ordinary barcode, and the red line on the right is the 5-digit extension code:

After setting to "Read 2-digit extension code" or "Read 5-digit extension code", the scanner can read a new barcode composed of a normal barcode and an extension code; it can also read a normal barcode without an extension code. After setting to "Don't read 2-digit extension code" or "Don't read 5-digit extension code", the extension code part of the new barcode composed of the ordinary barcode and the extension code will not be read, but the part of the ordinary barcode can still be read normally.
